Almost every website could benefit from a client feedback form. Client feedback forms allow you to collect valuable feedback that can be used to improve your products, services, or website. While there are many ways to add a client feedback form to your WordPress site, we’ll show you an easy way to do it with the WPForms plugin.
WPForms is the most user-friendly form builder plugin for WordPress. You can create a client feedback form in just a few minutes, without having to write any code. Plus, WPForms comes with a powerful Form Builder that allows you to drag and drop form fields to create your client feedback form.
Once you install and activate WPForms, you’ll need to create a new form. On the WPForms Form Builder page, you’ll need to give your form a name and choose the Simple Contact Form template.
After that, you can start customizing your form fields. WPForms allows you to add multiple field types to your form, including single line text, paragraph text, dropdown menus, checkboxes, and more.
For a client feedback form, we recommend including the following fields:

You can also use the Form Builder to add unique form fields, like a file upload field or a signature field.
Once you’ve added all of the fields you want to include on your form, click on the Save button to store your form.
Now that your form is created, it’s time to add it to your WordPress site. WPForms makes it easy to add forms to your posts, pages, or sidebar widgets.
If you want to add your form to a post or page, simply edit the post or page where you want to add the form. Then, click on the Add Form button, which is located above the WordPress editor.
A popup window will appear, allowing you to select the form you want to add to your post or page. Select your client feedback form and click on the Insert Form button.
Your client feedback form will now be added to your post or page.
You can also add your form to a sidebar widget. To do that, go to the Appearance » Widgets page and add the WPForms widget to a sidebar. Then, select your client feedback form from the dropdown field and click on the Save button.
Your feedback form is now added to your sidebar and is ready for visitors to use.
Whenever someone submits your client feedback form, WPForms will store the form entry in the database and will also send you an email notification. You can view all of your form entries by going to the WPForms » Entries page.
From here, you can view, edit, or delete individual form entries. You can also export your form entries to a CSV file.
